We are accepting new patient registration to our clinic.
When you have booked an appointment at our clinic, we will email you a confirmation of appointment form. We ask that you check to ensure the date and time of your appointment and practitioner and to contact us immediately in the event that you do not receive a confirmation, or if the details are not correct.
Included in the confirmation email will be the contact details we hold for you. Please click on the links to update the email address, phone number, correspondence address and GP details that we hold for you. We ask that you complete a medical history questionnaire prior to attending our clinic and you can follow the secure links in your confirmation email to complete this ahead of time.
While we do not need a referral in order to book an appointment for you, if you are using private health insurance your insurer may require that you get a referral. Please bring the referral along to your appointment so that our Dermatologist can review the information provided and correspond with your GP or Specialist who has made the referral.
The information we will request from you in order to register as a patient is:
- First name and last name
- Date of birth
- Mobile phone number
- Email address
- Postal address
For those patients who wish us to send a copy of medical reports to your GP:
- GP Name
- GP Surgery Name
- GP Surgery Address
- For those patients who wish us to invoice their private health insurer:
- Name of your insurer
- Membership number
- Authorisation code
